Proofing Behaviors Canine Obedience (James E Salvino): cannot make any guarantee regarding the outcome of any training/treatment plan programs. The owner assumes full and complete responsibility for:
Lesson Plan / Treatment Plan (s): The trainer is responsible for training the owner to train the dog (s) in all behaviors described in the Lesson Plan /Treatment Plan (s) listed in contract. The owner understands that they are being trained to train their dog (s) and that without 100 percent cooperation, as well as consistent reinforcement of Lesson Plan /Treatment Plan (s) the dog (s) will not be trained. The owner is ultimately responsible for the dog’s final outcome.
Cancellations: By Owner may postpone any lessons providing 24 hour notification to Trainer. In emergency situations including but not limited to : death, accident, illness the owner is required to notify within a reasonable amount of time.

Termination of Training: The Owner may terminate training /treatment plan at any time providing 24 hour notification to Trainer with no reimbursement of all money paid to lesson or treatment plan. In emergency situations including but not limited to : death, accident, illness the owner is required to notify within a reasonable amount of time.
Liability: I, as legal owner of pet (s) as stated in Contract have carefully read and fully understand this agreement, do hereby waive and release James E Salvino from any liability of any nature. This may include but not limited to injury, death, sickness or damage my pet may suffer during and after any training/treatment plan (s). I also indemnify and hold harmless James E Salvino from any and all claims due to but not limited to damage
the pet (s) may cause any family member or third parties during or after training.
Miscellaneous: The Owner is responsible for purchasing all necessary equipment that the Trainer recommends for training for the dog (s).
In the event that the Trainer; James E Salvino deems it necessary to obtain legal counsel to protect his rights due to but not limited to; injury, death, illness, the Owner of the Dog (s) agrees to pay all expenses incurred by James E Salvino his rights under this agreement including but not limited to cost and reasonable attorney’s fees.
This training agreement represents the full and only agreement between both parties. The Terms and Conditions, COVID19 Policy and Procedures, Safety and Health Policy and Procedures, Audio, Picture and Video Release form set forth in this agreement cannot be modified or changed in any way. Any alterations or addendums to this agreement will not be valid or binding, nor will affect the validity of any non-altered portion of this agreement unless signed by both parties. I have fully read, understand, and agree to the above Terms and Conditions.

Classifying Worker Exposure to SARS‑CoV-2
Worker risk of occupational exposure to SARS-CoV-2, the virus that causes COVID-19, during an outbreak may vary from very high to high, medium, or lower (caution) risk. The level of risk depends in part on the industry type, need for contact within 6 feet of people known to be, or suspected of being, infected with SARS-CoV-2, or requirement for repeated or extended contact with persons known to be, or suspected of being, infected with SARS-CoV-2. To help my clients determine appropriate precautions, OSHA has divided job tasks into four risk exposure levels: very high, high, medium, and lower risk. The Occupational Risk Pyramid shows the four exposure risk levels in the shape of a pyramid to represent probable distribution of risk. Most American workers will likely fall in the
Very High Exposure Risk
Very high exposure risk jobs are those with high potential for exposure to known or suspected sources of COVID-19 during specific medical, postmortem, or laboratory procedures.
High Exposure Risk
High exposure risk jobs are those with high potential for exposure to known or suspected sources of COVID-19.
Medium Exposure Risk
Medium exposure risk jobs include those that require frequent and/or close contact with (i.e., within 6 feet of) people who may be infected with SARS-CoV-2, but who are not known or suspected COVID-19 patients. In areas without ongoing community transmission, workers in this risk group may have frequent contact with travelers who may return from international locations with widespread COVID-19 transmission. In areas where there is ongoing community transmission, workers in this category may have contact with the general public (e.g., schools, high-population-density work environments, some high-volume retail settings).
Lower Exposure Risk (Caution)
Lower exposure risk (caution) jobs are those that do not require contact with people known to be, or suspected of being, infected with SARS-CoV-2 nor frequent close contact with (i.e., within 6 feet of) the general public. Workers in this category have minimal occupational contact with the public and other coworkers.
Jobs Classified at Lower Exposure Risk (Caution): What to Do to Protect Workers
For workers who do not have frequent contact with the general public, employers should follow the guidance for “Steps All Employers Can Take to Reduce Workers’ Risk of Exposure to SARS-CoV-2,” on page 7 of this booklet and implement control measures described in this section.
Personal Protective Equipment (PPE)
When selecting PPE, consider factors such as function, fit, decontamination ability, disposal, and cost. Sometimes, when PPE will have to be used repeatedly for a long period of time, a more expensive and durable type of PPE may be less expensive overall than disposable PPE. Each employer should select the combination of PPE that protects workers specific to their workplace.
Workers with medium exposure risk may need to wear some combination of gloves, a gown, a face mask, and/or a face shield or goggles. PPE ensembles for workers in the medium exposure risk category will vary by work task, the results of the employer’s hazard assessment, and the types of exposures workers have on the job.
High exposure risk jobs are those with high potential for exposure to known or suspected sources of COVID-19.
Very high exposure risk jobs are those with high potential for exposure to known or suspected sources of COVID-19 during specific medical, postmortem, or laboratory procedures that involve aerosol generation or specimen collection/handling.
Personal Protective Equipment (PPE)
Most workers at high or very high exposure risk likely need to wear gloves, a gown, a face shield or goggles, and either a face mask or a respirator, depending on their job tasks and exposure risks.
Those who work closely with (either in contact with or within 6 feet of) patients known to be, or suspected of being, infected with SARS-CoV-2, the virus that causes COVID-19, should wear respirators. In these instances, see the PPE section beginning on page 14 of this booklet, which provides more details about respirators. PPE ensembles may vary, especially for workers in laboratories or morgue/mortuary facilities who may need additional protection against blood, body fluids, chemicals, and other materials to which they may be exposed. Additional PPE may include medical/surgical gowns, fluid-resistant coveralls, aprons, or other disposable or reusable protective clothing. Gowns should be large enough to cover the areas requiring protection. OSHA may also provide updated guidance for PPE use on its website: www.osha.gov/covid-19.
NOTE: Proper disposal of PPE and other infectious waste must be followed.

Note Existing Clients from December 1 2019 to May 1 2020 will receive refund if Termination of Training is Required due to Covid 19 , payment will be received by client within in 30 working business days
Note due to medical issues James E Salvino will not wear a mask or gloves when training or interacting with clients and their dog(s).
Note : This Refund will expire June 30, 2020

Proofing Behaviors Canine Obedience LLC may use Audio Pictures and Video for situations including but not limited to Education, Health, Safety, Training and if necessary Legal purposes of Client, their dog(s) , Trainer, home , training facility and or open spaces related to state, county, borough.
The client releases all legal right to said Audio, Pictures and Video as said property and control to Proofing Behaviors Canine Obedience LLC pertaining to situations including but not limited to Audio Pictures and Video taken during lesson (s), Post on Facebook Instagram or any social media outlet
Refusal to sign date release form may or will be grounds for termination of contract at any time as per Terms and Conditions by Proofing Behaviors Canine Obedience LLC .
It is the responsibility of the Client of Children under the age of eighteen to remove a child or children in any Audio Pictures and Video.
Proofing Behaviors Canine Obedience LLC will never print or distribute any Audio Pictures and Video of minor(s) pertaining to situations including but not limited to Audio Pictures and Video taken during lesson (s), Post on Facebook Instagram or any social media outlet.
